A field trip permission slip detailing the day, time, place, event and chaperones should be completed and signed by a parent / guardian at least two days before any field trip and returned to the teacher/chaperone.
Verbal permission is not accepted.
However, if your child forgets to bring home a permission slip for a specific field trip, you may print out the 'generic' form below and fax to 982-5321 or scan and email to schooloffice@montedelsol.org
